Add JV URL to affiliate request page

Now vendors can provide their affiliates with a JV URL that can be shown on the affiliate request page.

This is great if you do not want to use, or in conjunction with, the built-in Marketing area of PayKickstart and you want to have your own page where you provide more details on your affiliate program.
